Report a Death

When reporting a death, please be prepared to verify your identity (including your name, date of birth, and Social Security number) and the identity of your loved one. It is helpful if you can provide the Navy Mutual policy number.
To Report a Death:
- By Phone: Please call 800-628-6011. Our hours of operation are Monday through Friday, 8:30 a.m. ET to 6:00 p.m. ET.
- Online: Please complete the form below. Once all required information is received, a condolence package will be mailed to each named beneficiary within five (5) business days. If additional information is required to process a claim, we will contact each beneficiary directly.
Please be prepared to arrange for the submission of a copy of the death certificate to Navy Mutual. Following receipt of the below report, we will provide secure transmission options to you for the purpose of electronically submitting the death certificate and any other relevant information. Please note that an original death certificate submitted to Navy Mutual by mail may be required to complete processing of a claim.
